This appears to be some kind of test. I wondered if anyone knew anything about it.

Yes, does look like a test...at first glance i thought it might be an induction questionaire. But seems more likely a survey of units and their level of training and how, if any, observances have been changed since marching in. It could even be a test for instructors (unlikely) to see how well they are doing their job.
Thought question and answer 3 was a bit dodgy. A service member SHOULD question an order if he/she beleives the order to be illegal.

In general it is assumed all orders are lawful unless it is specifically stated otherwise, there is generally a follow up question concerning a soldiers right to refuse an unlawful order. The questionaire looks like it is for an oral board for promotion or advancement through boot camp. The "S" and "US" denote "Satisfactory" and "Unsatisfactory" accordingly.

Looks like a Texas National Guard inspection / review form. Soldiers would be randomly quizzed on their knowledge by the inspector in addition to a review of their uniform etc. Not uncommon and done alot starting in bootcamp. The results are marked satisfactory (S) or unsatisafactory (US) and remarks made appropriately.
