Where did my Uncle serve?

    He was Maurice Albert Herbert
    WO 422/91803. Service Number 14241335
    All I know is that he served in tanks and I believe he fought at Monte Casino.
    In his later years he would have nightmares that his tank was on fire and he had to get out - there is some bad event in his past.
    Sadly he is no longer with us and although I have requested a copy of his Service Record there is a backlog of 12 months.
    Can you help? What information can we glean from the attached pictures?
